I've never worked with actual ivory, but what I do to polish my polymer clay pieces is this. It may work with your piece.
I get some fine-grade wet/dry sandpaper from the auto store. I wet it and start at 800 then go to 1000, 1500, and 2000 grit. By then the piece is usually silky smooth.
Then, I have a small bench grinder fitted with an unstitched muslin buffing disc. Put a box behind it in case the piece goes flying. Wearing a simple dust mask, turn the machine on and holding the piece on the lower half of the wheel, begin buffing. Keep the piece moving at all times so that the buffing is even.
